At Community Care City of Kawartha Lakes, we are committed to protecting your personal health information. Our policies and processes are reviewed for compliance with the Personal Health Information Protection Act (PHIPA) and we implement information management practices that protect your privacy. We collect and use your personal health information in order to provide you with the best care possible. You will be made aware of the purposes of collection, use and disclosure of your personal health information. At any time, you may provide us with direction regarding disclosure.
