Community Health Centre – a commitment to health promotion
Many recognize that there is a direct link to health and well-being with lifestyle choices. Further, there is a direct link to health care costs with those who make poor life-style choices or who may be impacted by economic deprivation. Health Centres are committed to incorporating health promotion as a component of their service basket. This can take several forms. It may be that through the multi-professional team, timely access to additional support services enhance capacity to cope and support behavioural changes. It might be group classes, on such topics as nutritional food preparation led by the Health centre dietician in a fun and stress-free environment. It could be helping to make necessary connections within the community through the social worker – raising awareness through education, making connections, supporting behavioural changes, nurturing accountability in a blame-free environment – these are just a few of the ways by which health promotion occurs.
The Community Care City of Kawartha Lakes CHC is committed to working closely as well with community partners and resources, to make sure that its clients have every opportunity to advance their own health and well-being. Programs such as Community care’s Falls Prevention, Cardiovascular Health Awareness Program and other community support services are designed to enhance the opportunity to promote health and ultimately independence and capacity of clients.
