Home At Last
This free service helps aging adults and adults with special needs settle in at home safely and comfortably in a timely manner after a stay in hospital. This transition service may include driving or accompanying the person home from hospital, picking up medications and/or groceries on the way home, preparing a light meal, providing personal care and/or homemaking services, following up with phone calls or visits and referrals to other community support services
