Hospital to Home (formerly known as Wrap Around)
This service, a partnership between Community Care, Ross Memorial Hospital and the Central East Community Care Access Centre, is designed to eliminate barriers to discharge from hospital. People who could be safely discharged sooner if additional support, service, equipment or retrofit is more readily available upon discharge can be considered for assistance. Referrals to Hospital to Home service, which is free to the client, are made by a multi-disciplinary team after assessment to determine that other services have attained maximum levels and the needed support is not accessible in the community by a publicly-funded agency.
